What to Bring During Your Visit

Requirements for Adults (16 years and older):

  1. COMPLETED DS-11 APPLICATION

  2. ORIGINAL OR CERTIFIED PROOF OF U.S. CITIZENSHIP and PHOTO COPY OF U.S. CITIZENSHIP

    You must submit one of the following:

    • Previously issued U.S. Passport
      Must be: undamaged & fully valid (10 years for adults)
    • Certified U.S. birth certificate (must meet all of the following requirements):
      • Issued by the City, County, or State of birth
      • Lists bearer's full name, date of birth, and place of birth
      • Lists parent(s) full names
      • Has date filed with registrar's office (must be within one year of birth)
      • Has registrar's signature
      • Has seal of issuing authority
      • California and Texas abstracts are not acceptable
    • Naturalization Certificate
    • Certificate of Citizenship
    • Consular Report of Birth Abroad or Certification of Birth

    NOTE: All original citizenship documents are required to be sent with your passport application. Originals of all documentation submitted, will be returned to you by the U.S. Department of State.

  3. PROOF OF IDENTITY

    Acceptable identification with photograph must be presented at the time of application. Additionally a photocopy of the front and back of each ID document presented when you apply is required.

    You must present one of the following:

    • Fully-valid Driver's License
      Out-of-state ID: If you submit an out-of-state primary identification, you must present an additional ID, as well.
      For example, if you apply in California with a Virginia Driver's License, you must present a second ID containing as much of the following information as possible: your photo, full name, date of birth and the document issuance date.
    • Current Military ID (military and dependents)
    • Previously-issued, undamaged U.S. passport
    • Certificate of Naturalization or Citizenship
    • Current Government Employee ID (city, state or federal)
  4. PASSPORT PHOTO AND PHOTOCOPY SERVICES

     We provide passport photo services for $17.00 and photocopies for $1.00 per page.

  5. APPEAR IN PERSON

    All applicants must appear in person.

  6. PAYMENT FOR ACCEPTANCE FEES

    Passport applications require two separate fees:

    1. Fee one: Paid to San Diego State University and is payable by cash, money order, cashier’s check, debit or credit. Personal checks are not accepted.
    2. Fee two: Payable to the U.S. Department of State only by check or money order (Please note the U.S. Department of State does not accept starter checks).

Passport Applications for Minor Children

  1. COMPLETED DS-11 APPLICATION

  2. PROOF OF U.S. CITIZENSHIP and PHOTO COPY OF U.S. CITIZENSHIP

    You must submit one of the following:

      Must be: undamaged & fully valid (5 years for minors)
    • Certified U.S. birth certificate (must meet all of the following requirements):
      • Issued by the City, County, or State of birth
      • Lists bearer's full name, date of birth, and place of birth
      • Lists parent(s) full names
      • Has date filed with registrar's office (must be within one year of birth)
      • Has registrar's signature
      • Has seal of issuing authority
      • California and Texas abstracts are not acceptable
    • Naturalization Certificate
    • Certificate of Citizenship
    • Consular Report of Birth Abroad or Certification of Birth

  3. EVIDENCE OF PARENTAL RELATIONSHIP

    You must submit evidence that lists you as the parent of the child applying for a passport. Some documents such as a U.S. birth certificate, provide both evidence of citizenship and evidence of parental relationship. If you are submitting one or more of the documents below as evidence of U.S. citizenship too, you must submit a certified copy. 

    Examples of documents that may be used as evidence of parental relationship are:

    • U.S. birth certificate
    • Foreign birth certificate
    • Adoption decree
    • Divorce/Custody decree
    • Consular Report of Birth Abroad of a United States Citizen (FS-240)
  4. PROOF OF IDENTITY

    Each parent must provide an acceptable form of ID.

    When you submit your application, the parent (or child, if he or she has an ID in his or her own name) must present one of the following primary photo identification documents, and submit a photocopy:

    • Valid Driver’s License (plus a second ID if issued in a different state than where you apply)
    • Current, undamaged U.S. Passport (if issued less than 15 years ago)
  5. APPEAR IN PERSON

    Both parents/guardians must appear in person with the child and provide consent, authorizing passport issuance to the child. If one parent/guardian is unable to appear in person, then the DS-11 application must be accompanied by a signed, notarized Form DS-3053: Statement of Consent from the non-applying parent/guardian.

    If the child only has one parent/guardian, original evidence of sole authority to apply for the child must be submitted with the application in the form of a:

    • Court order granting sole legal custody to the applying parent (unless child’s travel is restricted by that order)
    • A court order specifically permitting the applying parent to apply for the child’s passport
    • Judicial declaration of incompetence of the non-applying parent
    • Death certificate of the non-applying parent

    NOTE: Photocopies and notarized copies are unacceptable.
